KVKK Information Request Form

GENERAL

In order to evaluate and resolve the applications to be made by you, the data subjects, pursuant to Articles 11 and 13 of the Personal Data Protection Law No. 6698 ("KVKK") promptly, effectively, and comprehensively, this Application Form has been prepared by Elif Cığızoğlu Moda Tasarım ve Tekstil Ürünleri Tic. Ltd. Şti. in its capacity as the data controller.

METHOD OF APPLICATION

As data subjects, pursuant to Articles 11 and 13 of the KVKK; you can submit your requests regarding the implementation of the KVKK to our Company, which bears the title of data controller, in writing by filling out this form or by other methods to be determined by the Board:

By applying in person or sending via notary public a signed copy of this data subject application form to the address "Harbiye Mahallesi Teşvikiye Caddesi Teşvikiye Palas No:23/6 Şişli/İstanbul", or by sending a signed copy of this form to the e-mail address info@elifcigizoglu.com using the electronic mail address previously notified to the data controller by the relevant person and registered in the data controller's system.

Regarding your application to be made pursuant to the relevant article of the KVKK, we kindly ask you to fill out the following information completely so that we can identify you and so that the necessary research, evaluation, and analysis can be carried out by Elif Cığızoğlu Moda Tasarım ve Tekstil Ürünleri Tic. In order to ensure the security of your personal data, within seven (7) days from the date your information request reaches our COMPANY, our COMPANY may contact you to confirm that you are the data subject and may request some information and documents from you in this regard. The information and documents you provide to us within this scope will be destroyed immediately after your identity as the data subject is confirmed.

In case the requested information and documents are incomplete, the information and documents must be completed and submitted to us upon our request. The thirty (30) day period specified in Article 13/2 of the KVKK regarding the conclusion of the request will be suspended until the information and documents are fully submitted to us.

I request that my information application made in accordance with the KVKK be evaluated and concluded within the framework of the request/requests I have stated above, and I accept, declare, and undertake that the information and documents I have provided to you in this application are accurate, up-to-date, and belong to me.

I allow the information and documents I have provided in this application form to be processed by your Company limited to the purposes of evaluating and answering the application I have made pursuant to Article 13 of the Personal Data Protection Law No. 6698, delivering my application to me, and determining my identity and address.

This application form has been drawn up for your requests arising from the rights listed in Article 11 of the Personal Data Protection Law No. 6698. These requests are carried out within the scope of Articles 11 and 13 of Law No. 6698 and the "Application Procedure" in Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller.
